Below is a draft of the Self-Awareness Assessment that can be emailed, texted, or mailed to someone you trust. This is a completely optional exercise. Feel free to use it in its entirety or revise based on your own goals.

Hope you’re doing well. Would you take a few moments to help me with a self-awareness exercise? I’m sending this short list of questions to folks that I trust to help me understand how I impact the world around me. All of the questions are neutral. Your answer could be positive or constructively “negative.” I trust you to be as honest as possible so that your input can do the most good. Would you answer the questions below and send them to me as soon as you can? I completely understand if you don’t have the time to do this. Thank you!

1. What is a person most likely to experience when they encounter me?

2. What are the usual things you can count on from me?

3. What are the things that you can seldom count on from me?

4. Is there something about me that you think I don’t know yet?

5. Based on your interactions with me, what is the thing (or things) you think matters most to me?
